We provide two affordable service options tailored to your needs, with an additional service available for caregivers. Rest assured, we make the process as smooth and straightforward as possible.
Option 1: Full-Service Package
$200 – Everything You Need!
This option includes:
Office Visit – Consultation with our physician.
State Fee – The required $50 fee to the state.
Paperwork Submission – Our staff will handle all the paperwork and submit it directly to the state on your behalf.
Option 2: Do-It-Yourself Package
$125 – You Handle the Paperwork.
This option includes:
Office Visit – Consultation with our physician.
State Fee – You’ll be responsible for the required $50 fee to the state upon submission.
Paperwork – You’ll be responsible for submitting your paperwork to the state either by mail or online.
Option 3: Minor and/or Caregiver
$337 – We Handle the Paperwork!
This option includes:
Office Visit – Consultation with our physician.
State Fee 1 – The required $50 fee to the state for the Caregiver.
State Fee 2 – The required $50 fee to the state for the Minor or Applicant.
Background Check– The $87 Caregiver criminal background check required by the state.
Paperwork Submission – Our staff will handle all the paperwork and submit it directly to the state on your behalf.
